Cigna Updates Authorization Policy for CTA and FFR-CT Analysis

Arlington, VA (January 29, 2021) — Effective February 1, 2021, Cigna will no longer require pre-authorization for Computed Tomography Angiogram (CTA) of the heart, coronary arteries and bypass grafts with contrast material, including 3D image post-processing, (including evaluation of cardiac structure and morphology, assessment of cardiac function, and evaluation of venous structures, if performed). The change removes pre-authorization requirements for CPT code 75574 in all markets with Cigna eviCore healthcare except for Hawaii, Puerto Rico and Guam.

Cigna also removed preauthorization, effective February 1, for Fractional Flow Reserve-Computed Tomography (FFR-CT). See Cigna's Master Pre-certification list for providers, page 187.

“The favorable policy update shows that Cigna recognizes the use of CTA and FFR-CT as a front-line test which can lead to improved patient outcomes.” says Dustin Thomas, MD, FSCCT, Chair, Advocacy Committee, Society of Cardiovascular Computed Tomography (SCCT)

For coverage under this new policy patients must have both of the following:

  • Stable chest pain
  • Intermediate risk of coronary artery disease (e.g., suspected or presumed stable ischemic heart disease)

Per the updated policy, “Physicians will be able to order a CTA and FFR-CT Analysis for all Cigna beneficiaries when medically necessary without obtaining prior authorization.”
